Admission Criteria

Providing outcome-focused support for individuals

Integra will provide support and rehabilitation for individuals:

  • aged over 18
  • with a primary diagnosis of an enduring mental health disorder including schizophrenia and personality disorder
  • with an acquired brain injury (although this is only in some homes)
  • who may have complex needs and/or a secondary diagnosis such as learning disability, cerebral palsy or multiple sclerosis
  • whose current condition is such that it is difficult for them to function successfully and independently in the community without additional residential support and rehabilitation
  • who do not require physically secure accommodation
  • who are ambulant, although most of Integra's homes have been adapted to accommodate individuals with mobility issues should the need arise

Integra does not offer a service for individuals:

  • whose admission, following joint assessment, is considered not to be a good fit with the existing mix of individuals who have chosen to live in the home
  • who require an emergency admission
  • with a primary diagnosis of substance or alcohol misuse

Integra's services are aimed towards individuals who will either:

  • benefit from a recovery and rehabilitation focused placement (designed to meet agreed individual outcomes within specific time parameters) or
  • have been assessed as needing a longer term residential care placement focused on maximising independence and quality of life

We focus on effective partnership working with the client, their family and the multi-disciplinary team in order to achieve agreed goals and, where relevant, assist individuals to move on to live independently (or with visiting support).

Integra tailors the support provided to each individual in line with their assessed need and our fees reflect the current needs of the client, which often reduce as placements progress. We offer the ability to provide transitional or ongoing relational support via our domiciliary care services (including supported accommodation).

Referrals

Integra welcomes referrals for men and women over 18 with a mental health diagnosis and/or an acquired brain injury. 

We have residential care, supported accommodation and outreach services in Cardiff, Carmarthenshire, Neath, RCT and Swansea.
