Providing a prompt responsive assessment of potential referrals
In order to ensure that each client receives support and rehabilitation specifically designed to meet their individual needs, a detailed assessment is carried out by the home manager and a clinician prior to admission.
Integra does not meet a potential client until this is approved by their clinical teams, and where relevant, by their commissioning authority.
All individuals considering a placement at Integra will have the opportunity to visit the potential home, meet other clients and staff and have a period of graded exposure to the home prior to an admission.
We confirm we are an approved/accredited provider for the National Collaborative Framework in Wales, Cardiff and Vale University LHB, Bristol County Council and for the West Wales commissioning hub.
We are currently accepting referrals for all of our homes in Cardiff, Carmarthen, Llanelli, Neath Port Talbot, RCT and Swansea and request that you contact our Service Coordinator by phone (02920 49 44 45) or email if you have a potential referral or wish to discuss whether we may be able to meet the needs of a specific individual.
